Here’s how to use the IVF Medication Log Sheet during your upcoming IVF Cycle

Medications – Fill in one medication per box in the left-hand column, and use multiple sheets if needed. 

Cycle Day Boxes – Fill in your am/pm dosages per box.

Helpful tips for your IVF Cycle

If you want to completely organize your cycle with this log sheet, use highlighters or symbols to designate when you have to return to your fertility center for blood work and/or ultrasound.

Try to look ahead at how much medication you have and compare it to your dosages to see if you need refills ahead of time. Avoiding last-minute panic can help reduce some degree of worry and empower you to find opportunities for wellness and self care during your cycle.
